I want to disable the remaining uncheck box when I check some of them. That means if I check any two boxes and get its value then the other left uncheck box will become disable. I have made a function to get the result but I am getting this that if I check one box then all the boxes become disable.

You can just use some css selectors to accomplish most of this.
Here is how you can get your selected size:

I have a list of 5 radio buttons, with the last radio button to select a manual entry text (an "other" option). I can't seem to get the form to work for all 5 possibilities - either it returns the radio value or the written value (currently only the written value and not any of the preset radio values). Can't get it to work for all of 5 possible returns.

You can write a single function for your form which checks for two events:
radio button check
text input blur
and then gives an appropriate response based on which radio button registered a check event or whether the text input registered a blur event:

I'd group your inputs in a container and watch that for events using addEventListener. Then loop through the checkboxes, checking their status. Finally set the button to disabled unless our criteria is met.

Few suggestions
1.Always id should be unique. HTML does not show any error, if you give multiple objects with the same id but when you try to get it by document.getelementbyid it always return the first one,because getelementbyid returns a single element
when there is such requirement, you should consider having a classname or searching through the element name because getelementsbyclassname/tag returns an array
Here in the markup i have added an extra class to query using getelementsbyclassname
To avoid adding extra class, you can also consider doing it by document.querySelectorAll

The markup, you can also add a click listener to the form to do updates rather than on each radio button. You should also have a reset button so the user can clear the form.
